Voluntary Health Insurance

• Chronic or pre-existing illnesses and their treatment, whether or not the insured person was aware of them, as well as illnesses caused by alcoholism or drug addiction.
• Experimental treatment, infertility, alcoholism, substance abuse, drug addiction, dental treatment, infectious or sexually transmitted diseases, HIV/AIDS and related complications.
• Oncological, psychiatric and tuberculosis conditions, diabetes mellitus, chronic kidney failure, hereditary or congenital diseases and other excluded chronic conditions.
• MRI, CT scans, scintigraphy and other high-tech investigations; HIV/AIDS tests, hepatitis B, C and D, sexually transmitted infections and similar markers.
• Planned surgery, cosmetic or plastic treatments, sex reassignment, pregnancy, childbirth and related complications, abortions, curettage, prostheses, implants, glasses, lenses or vision correction.
• Suicide, self-harm, war, revolution, state of emergency, hostilities, insurrection, riots, terrorism, biological or chemical contamination, or enrolment in armed forces.

• The insurance is valid only within the Republic of Moldova and only for the medical services included in the package specified in the policy.
• Outpatient care is provided in polyclinics or state public institutions; inpatient care is covered only if expressly stated in the policy, in state hospitals, excluding IMSP of ACSR.
• For inpatient treatment, emergency hospitalisation is compensated from day three for up to eight days; intensive care is covered for up to three days during hospital treatment.
• As a rule, the insured person pays for medical services and then requests reimbursement; for hospitalisation, policy validation and direct payment or transfer of costs to the medical institution may be requested.
• Compensation cannot exceed the insured sum and the package limits; expenses are accepted only based on original medical and payment documents.
• Incomplete or inaccurate information regarding health status, failure to submit documents, delayed notification or treatment of an excluded condition may lead to full or partial denial or termination of the contract.

Only in the Republic of Moldova, in state public medical institutions: polyclinics or outpatient facilities for the minimum package, and state hospitals for the standard minimum package where inpatient cover is stated in the policy.

• At policy inception, fully and accurately declare your health status, risks and material circumstances; read the contract, policy conditions and insurance programme.
• Pay the insurance premium in full in the amount and by the deadline stated in the policy; allow the processing of personal data required for the insurance and the claims file.
• In the event of a sudden acute illness, seek medical care from an IMSP or another state public institution and follow the doctor's instructions.
• Notify the insurer or Claims Department at (022) 78 38 00; in case of hospitalisation, notify the insurer immediately and request policy validation.
• Pay for medical services where the package provides reimbursement and keep original receipts, invoices and medical extracts.
• Within 14 calendar days after treatment ends, submit the claim with the ID document and copy, the policy, the medical extract or observation sheet, the hospitalisation referral, original receipts and any other requested documents.
• Authorise medical institutions and doctors to provide the insurer with the information needed regarding health status and confirmation of the insured event.

The premium is paid in full, in the calculated amount, at the place and within the deadlines stated in the policy. The price depends on the selected package, the insured sum, the policy term and the insured person's age in full years on the date the premium is collected. The tariff includes a minimum package with an insured sum of MDL 7,000 for 3, 6 or 12 months, and a standard minimum package with an insured sum of MDL 20,000 for 6 or 12 months.

Cover starts from the moment the premium is paid, but not earlier than the date stated in the individual policy issued to the insured person. Only sudden acute illnesses arising during the valid insurance period are covered. Cover ends when the period stated in the policy expires, upon termination, rescission, invalidity or in other cases provided by the contract and the law.

Either party, the insured person or the insurer, may initiate termination before the insurance period begins by giving written notice to the other party at least 30 calendar days before the termination date. In the event of termination, the insurer refunds the premium after deducting administration expenses. The contract may be amended only by mutual written agreement, and disputes are settled amicably or by the competent court.
